*REGISTRATION FORMS*
Please wear comfortable, flat, non-slip shoes. Dogs need a properly fitted collar and six foot leash. These items are available for purchase at class. Dogs MUST have current vaccinations and be free of parasites. Please plan to bring proof of current vaccinations with you to the 1st class, unless you have provided it at the time of registration (see below).
To complete the class registration process, the Policies, Hold Harmless and Registration forms should be downloaded, printed, completed and sent along with PROOF of VACCINATIONS via one of the following methods:

Please make checks payable to: "DOG TIRED DAY CARE"
and mail with registration forms if pre-paying, or plan
